Read the Steps Below to Make Your Transition into PT Here a Smooth One.
1) Open and print each of the pdf forms listed on the Physical Therapy Forms page. If you don't have a printer, skip steps 1 & 2, and go to step 3. Please come 20 minutes early for your initial appointment so we can give you this paperwork to complete when you arrive.
2) Read, complete and sign each of the forms that require information and a signature. Bring these forms with you when you come in for your first appointment.
3) Go to the "Registration Form" page by Clicking Here. Complete and submit the online registration form. We will contact you shortly after the submission to help you schedule your physical therapy appointments.
4) On the day of your initial evaluation, please arrive 10 minutes early with your completed paperwork so we may review it with you and give you copies if needed. Thank you.
